Need to solve this problem

Hey guys, I was hoping I could have some help with this problem since I am not very good with vb,

In a data entry form program, how do you loop a program to ask the user to input their name, tel no that kind of thing. for example a questionnaire askes you to input your details, once you put them in and enter them via command button, it will reset the process and will be ready for new user
